How to use this page: Forms are ordered to follow the degree timeline for the typical student. Please contact the Academic Services Office if you need additional clarification on a policy or if there is any difficulty locating information for a form.
ME Declaration Form
The ME Declaration Form is for students who have completed the necessary requirements and are ready to declare Mechanical Engineering as their major. This form contains the Declaration Form and the Long-Term Degree Plan and should be submitted after the third week of the semester to the Academic Services Office. Students should fill out the Declaration Form, review the degree requirements, and come up with a tentative Long-Term Degree Plan. Return this form via an e-mail to me-aso@umich.edu or in person at 2206 GG Brown. An advisor will review your degree plan and let you know if any changes need to be made or if they would like you to come in for further discussion.
Energy or Manufacturing Concentration Application Form
Please fill out this Concentration Application Form and email it to me-aso@umich.edu or turn it in directly to the ASO in 2206 GG Brown if you would like to concentrate in Energy or Manufacturing Systems.
Petition Form
Any student can petition for modification of academic policies. The petition form is submitted as an email attachment sent to me-aso@umich.edu, or dropped off as a hard copy to the ME ASO office (2206 GG Brown). Students should always attach supporting documentation (e.g. a course syllabus) to strengthen their case. The result of the petition will be communicated via email.
If you are petitioning a program (i.e. Mechanical Engineering) then the ME ASO will review and make a decision. If you are petitioning a College rule both the program and the College will review the petition. Depending on which parties are involved in the review process, the response time frame can be the same day, a few days, to a few weeks. The ME ASO will keep you informed via email.
Letter Request Form
The Letter Request Form is for students who need to confirm aspects of their academic career to an outside representative. The form should be returned to the ME Academic Services Office via an e-mail to me-aso@umich.edu or in person at 2206 GG Brown. Please allow 5 to 7 business days for processing of letters.
Sequential Graduate/Undergraduate Study (SGUS) Election Form
Students who are interested in completing a Sequential Graduate/Undergraduate Study program are required to submit the SGUS Election Form. The form confirms the ability of the student to complete the degree requirements.
